Don't smoke marijuana anymore. There's also a chance she doesn't remember it at all if everyone was under the effects of it.

Weak sauce, OP. You can work through difficult issues without drugs. If you're serious about getting past your fears, try something else. Two ideas: meditation and therapy are both good, especially in conjunction.

Mate, your fuck up was doing something new and unpredictable (drugs) in the presence of your LTR.
I won't tell you not to smoke ever again (though very compelled to do so based on your reaction to it), I've been smoking for a long time and neither me nor my friends have ever had a similar freakout, my advice is if you're ever smoking again to smoke with your friends not your LTR, until you're familiar with the high. Btw I choose not to smoke or at least not smoke too much when I'm with a girl, while alcohol loosens me up and removes some inhibitors, weed only makes us do stupid things, and if someone were to film you high, you'll see the traits we display while high are definitely not desirable by women. Only smoke with you buddies to relax and have a good time.
As for your LTR and the incident, well she won't forget what happened tell you that right off the bat. Not sure you should have apologised either, I would've just acted like it never happened.
Though what worries me is what she told you, she's already thinking about mariage and kids and how nice you are... Tread very very lightly. Can't point out what to do exactly, not very into LTRs myself, hope someone else chimes in.

Sounds like it got laced with something.
I know marijuana is a lot more potent than it was when I was a youth in the 70s, I was paying about $40 an ounce back then for Colombian. Nowadays you're paying a hell of a lot more, and from what I understand the stuff really kicks ass.

MJ is never laced with anything, lacing marijuana with a more expensive drug is plain stupid. Many claim it's laced with PCP or cocaine, smoking cocaine is waisting cocaine since it evaporates at 400 degrees, and if you had PCP you'd feel an out of body experience which you didn't feel, neither did your friends.
Again, you just freaked out on THC, don't go heroic dose if you only smoked 2 times prior, and don't do new drugs in front of your LTR
