"4 days in advance" doesn't sound like flaking, but more like she was really busy. Don't think too much and just choose a day when you're free.

More context would be helpful. I would just give ONE date that I will be free (asap) except the days she said she will be unavailable.

Say exactly the date you wanna meet.If she does it again just tell her that you do not appreciate games and that you should stopseeing eachother.If she tries to move it on another day you tell her you have something better to dofor example:"Sorry,but I cannot go out then because I have to go on a party(or whatever the case may be).".If she comes,you know what you need to do.
