I believe the proper reply would be "Thank you for relieving me of my nose, Sir." You laugh, but I'm actually going to do this. If combined with the aforementioned explanation about male-female greetings, then maybe segue into a conversation about gender roles and whatnot, I think it could work well. Or women will be freaked out by some strange guy suddenly lunging towards their faces. I don't think there's going to be any gray area.

I always find it a bit awkward shaking a girl's hand, period. It just seems like a very masculine greeting, but unless we're gonna revert back to bowing & courtesying or having men exclaiming Enchante! while kissing women's hands, it's all we really have. It seems like we really should have come up with a better male-female greeting by now. Fuck it, I'm just gonna do the whole "Got yer nose!" shtick whenever I'm introduced to a girl.

I've seen too many instances of guys like that going apeshit if they're confronted with someone questioning their motives in front of the girl and seemingly "revealing" that he's not just being helpful. No doubt. Again, it's how you confront them, and context is everything. Sarcasm definitely has it's place, but sometimes the best way to disarm someone with an attitude is with polite, honest questions about their behavior "Sorry, but don't you think she's capable of deciding how to interact with people?"
