No worries. The opener is of least importance. It's REALLY how you say. Today, I got my coaching student to say what mentioned here, he couldn't pull it off. His body language was totally off. The girl didn't want to shake his hand. Only after I adjusted his body language, his results changed. When I went over to another girl, and say the same thing, the girl stopped and we chatted.

Depends on what you mean by escalate. You don't have to touch a girl during the approach. Do some simple, high five fist bump, shake hand to break touch barrier. If you REALLY did assume attraction, you WOULD be doing things that is attractive to her. You build rapport by have a good conversation. There's no such thing as I forgot to build rapport, it probably that you are not focusing well on the conversation.
