Well, IANAL and all that, but I believe vaccines aren't generally mandatory, but vaccination as a requirement for enrollment in school and child care is upheld as legal. There's an obvious public health reason to require vaccination for enrollment in such programs, while no such public health justification exists for mandatory paternity testing. I think the best unconstitutionality argument is arguing from the right to privacy, which is seen as a 10th amendment protection.

I'd say the government requiring a medically unnecessary procedure definitely violates the 10th Amendment (which has been invoked to protect personal privacy in medical issues, most famously in Roe v Wade).
