Have you used tinder / traditional dating sites? Don't replace actual game with them, but they can be a good way to get in a bit of 'tuning' when you're not doing much else and have your phone there (e.g. Lunch break)

I'm terrible at opener's and if I have a good opener I'm terrible at holding conversation

Practice practice practice. Chat with as many people - not just girls but cashiers, bus drivers, janitors, old men and women, everyone. Learn from each one to see what you could have done better and remember, it takes two to have a conversation. You never know who could just be having a bad day and don't feel like talking. Watch some comedy movies or TV shows and pay attention to how people talk and how they use humor and stuff like that. Watch some stand up comedy (not really my thing but I heard it works).

One thing that I think is a mistake (which I may get severely downvoted for) is trying to figure out the structure of everything. Stop thinking along the lines of Open-Converse-Rapport-Kino-Escalate-Close. It's human interaction, not a scientific procedure lol. Don't get me wrong, it's great to know the theory behind these and consciously try to incorporate them into your interactions, but don't put so much weight on them, because part of your problem might be too much self monitoring. You don't need a good opener. My openers are usually "Hi, how are you?"
