None of these answers are brief.. but I'll give brief answers to start, I'm going in order matched with your bullet points:

There are a few ways to make more of an impact. Improving your aesthetic especially body, having more confident and masculine body language. But over all else I think the way you make an impact is by making the approach more authentic, say what you feel in that moment, and not exactly like any approach that came before.
Girls are throwing out little signs that they like what they see all the time. These are called IOI's or indicators of interest. They can either be done purposely, or she might not even know she's doing them. Basically, be hyper aware through life, and look for girls looking at you. If she looks at you for even a second it's an IOI.

the old 3 second rule is one of the best things the PUA world has ever spit out. I use a three breath routine my self. Breath 1: I blank my mind and focus just on my breath. Breath 2: I check her out in a sexual way and let myself get a bit turned on by her. Breath 3: I straighten up my body language and demeanor to be more confident. This routine get's my mind confident, slightly sexual, and not overthinking (therefor spontaneous.).

Addressing your first problem, be different! Girls aren't interested in the cookie cutter flirt because every guy with minimal pickup skills does this. They want someone to challenge and test them, so if something comes up where you would provide a normal answer: girl "how are you?" Instead of saying "good" or "fine" say something more along the lines of "I'm feeling extraordinarily fantastic!" Or something outrageous! Be careful not to go too far with this, though.
