How long should the gap of the day I ask her out and the actual date itself be?

It's not too important. Maybe a week ahead at the most, if possible.

Her exams will be over next Tuesday. Should I ask her out soon and arrange it for sometime later next week or should I ask her next week when she will be more relaxed after her exams?

Yeah, don't over-think it. I'd just ask her now, about next week. Give a specific time, place and activity.

She either says yes or no. If she says no, but really wants to hang out, she'll offer an alternative.
