A couple things:

How did this even come up that she said this?
She's basically saying that she's got an open disclosure policy with you. Though in practice, this really just means that they want you to think they're honest, or want more intimacy with you.
You wouldn't verbally ask someone if they're interested anyway.

What questions can/should i ask that will lead to a date/relationship?

There is no "question" that one "asks" verbally, that leads to a date or a relationship.

A date: You get to one of these in the following ways:
- You invite someone to hang out one on one. Mention a specific date, time, and place when you do.
- Bonus points if you choose something you're both interested in, or something light and fun.
- You don't use the word, "date."
A relationship: You get to one of these in the following ways:
- You hang out one on one with someone over the course of a month or so.
- Don't necessarily push sex early; it's okay to wait a smidge.
- By the 1-2 month mark or after sex, you might say what you want and ask what they want.

You'll note that none of these really involves some sort of question that you wouldn't be able to ask without her being 100% honest. This thing she has said doesn't, in itself, really constitute a short cut. It just makes it seem like she's willing to work with you! Which would make things easy, though.

Stop trying to agonize over the "right question." She just said she wants to be open with you, but that doesn't mean there's some question that unlocks her.

Just invite her to hang out and go from there.

You don't need 100% honesty to get this sort of information.

One assumes you have known this girl a bit. You know stuff about her. She's going to talk about things she likes and things she likes doing.

You use this stuff as you're thinking about what to suggest. It doesn't need to be earth-shattering. You don't need to reinvent the wheel.

You could grab coffee in a nice independent coffee shop with comfortable seating.
You could suggest hiking in a cool area.
Literally Google "interesting date ideas" + "the area you live in."

Honestly, one thing to do is take note of when the girls you know say they really had a good time on a date. Take note of:

Where they went
What they did
What the guy did

Girls will literally hand you information you can use.
