Those are definite IOIs, and you should have approached her.

Personally, I approach every woman I'm attracted to, whether or not I think she's giving me IOI's. I've hooked up with amazing women who gave me no IOI's before I approached them.

One fun way to open her since you kept catching her looking, would be to walk up to her and say: "Excuse me, I saw you checking me out. (Pause and smile here, build some tension before continuing) You look like someone I'd like to meet, too. I'm Wonderingrock, what's your name?"
