He is crying for help. At this point am guessing he really doesn't want to kill himself. He is having an impossible time swallowing the pill and is trying to force the round peg through the square hole. I wish him luck as the only solution is going to be him toughening up and not denying the harsh truth. It can be devastating when you struggle to deny what you see and the "professionals" diagnose you as though the problem is yours and push those meds on you.

The biggest sticking point is that being trapped in a marriage is more lonely and soul destroying than social isolation will ever be. The guy does have substance abuse problems, but I think his wife would be emotionally abusive to him no matter what the situation is.

I don't think the stigma we put on suicide is a good thing. In Switzerland anyone over 18 can opt for assisted suicide, you don't have to be sick. They won't ever allow that here, they need OBEDIENT WORKERS. So many people would be so much better off if they could opt for suicide legally, lets be honest, there are a huge % of people on this earth who's lives are only going to get worse, until the day they die of natural death, not better.
