People often transpose the terms respect and courtesy. Common courtesy should be given to everyone unless you have good reason not to. True respect, however, is earned as you noted, over time and through being worthy of emulation.

I don’t think it’s so much respecting a person you don’t know (after all, you’ll likely respect a general in uniform even if you’ve never met him before) but the real issue is respecting someone who doesn’t respect you. If you try to be polite with someone and they very clearly don’t give a shit about you then something needs to change: whether that be commanding respect, disrespecting them (an option but not usually the best one), or removing yourself from the situation

Sorry, but I disagree.
The first thing to understand is that respect is not a yes/no issue. Respect comes in degrees.
The second relevant point is that at I've never met even one person who had nothing to teach me. Every single person on the face of this planet knows something that I do not know. And as such has the potential to be my teacher. I don't go around disrespecting my teachers.
For me, every person starts with a certain basic level of respect. He can earn more respect by acting respectful and demonstrating merit. He can loose respect by being a douche or bad personality. Note that I will never disrespect someone for not knowing something. I was not born knowing everything and as long as a person is willing to learn, I will never disrespect him for his current education.

For me this is a continuous process. People have a chance to both redeem themselves and ruin pre-established respect. Sort of a weird social currency.
