The latter. As long as individuals foot the bill, governments have little incentive to change. If the governments are forced to pay, they might look seriously into why they should pay at all - perhaps even leading to changes in who can avail the services of the state. An end to the free run. Wishful thinking I suppose, but theoretically possible.

With the first one a man will be definitely destroyed.

We already suffer under the second one and at a certain point, change is inevitable in our favor. Although it might help a few men, it doesn't directly destroy a single man absolutely, like the first. The economy always adjusts to free handouts by becoming more expensive, forcing freeloaders to fend for themselves.
