The difference is whether you are doing something for yourself or for someone else.

When you hold a door open for an elderly person you are doing it because you would have felt bad if you didn't hold the door.

When a girl drops a pen and you pick it up to give it to her are you doing that for you? No. Let her pick up her own pen and make fun of her for being nervous around you.

Do you hold the door open for Men and Women?

Manners are fine.

Don't linger and wait to keep a door open for a girl 10 yards away if you wouldn't for a man.
