Sure if I think to myself that it doesn't matter, that I can do it, that nothing's gonna happen, then it gets pretty easy.
But like I said sometimes I just can't help but think about all that could go wrong.

And in most case scenario it will go wrong and its fine, but its life and you can live it or just hide in a shell.
Good point man. Gotta ignore all what could go wrong, and focus on what could go right. And always gotta keep in mind that regretting what could've happened is far worse that getting rejected or having an awkward conversation.

I'm not really sure about that. Usually when I see a guy approaching a girl I would just assume that he's cold approaching her or something. Why wouldn't people assume the same for me ?

First, cold approaching is so rare (I assumed you were talking about daygame) that most people will think a stranger approaching another means they’re friends or acquaintances. Or he’s trying to sell her something, or convert her to a religion.
Second, Why do you care what strangers think? You’ll never see them again. This really literally isn’t worth the mental energy you’re putting into it.
I approach around other people often - like in crowded places enough that others definitely notice me doing it and I notice them too. Of the 100s of times I’ve done this, only one negative experience comes to mind and it was because I opened her, got rejected, and opened a girl like 5 ft from her and was doing well with this girl. 1st Girl started yelling at me. it was so abnormal I brushed it off and kept talking to the girl I was already feeling - and we both agreed the girl was crazy.
