Yeah, that was one of my hangups, and it still is somewhat. I always felt that a gentleman does not force his attentions on a lady, he must be formally introduced. That's an old etiquette principle from the Victorian Era. Result: I never approached women. Fail.

Flip it the other way around. Introduce someone you know to someone else you know. Organize something yourself. Say you have a work colleague you can stand and you have someone you know from some night class you take. Invite them both out and introduce them to one another.

Relationships work on a rough reciprocity basis. When you meet someone new, think to yourself "do I know anyone I could introduce this person to" and then arrange it if possible. You'll find that people will return the favor.

Pretty much what everybody else said here, you gotta start making moves to fix the situation mang. I guess you could wait around till somebody does it for you but it seems kind of like wasted time if you want it. Ask a guy to spot you at the gym then go to beers afterwards. Join any class that interests you (ie. yoga, acting, improv, pottery, the fuck ever), Look up a group that meets every morning to go running or some shit, you know? Get out there and build your peeps one person at a time.

Go to bars, ask people where they are from, buy a couple a beers and go sit down and offer them to random strangers. Just have a good story to tell! It works in most cities, especially if the group looks inviting.
