How do I start a conversation?

I've never been scared to approach a girl, my problem with girls is that I just don't know what to say. How do I start a conversation with someone random? How do you strike up a conversation with someone at a bar or on the street without it getting awkward? And how do you close? Getting a number isn't a problem, but what do you do after that?
