At some point during the date, the conversation will most likely turn to an activity or event or even a place... at that point, mention loose plans to do/see/go to that thing/place sometime. Before the end of the date, bring it up again and figure out a time to do it. At the end of the date, say goodbye with something to the effect of "see you at suchandsuch".

Works for me! If no somewhat concrete plans are made by the end of the date, I assume its going nowhere and move on.

To answer your question:

Meet at a cafe near your place.
Get tea or something similar to go. (Usually makes for a very cheap date—two or three bucks—and there are even tricks to split the bill.)
Take her on a walk, sit under a tree to talk, or go to a somewhat fancy hotel lobby/lounge to chat.
When you're walking with the girl, walk so that you're walking way too close to her...so your arms are sort of touching. This makes things more romantic and sexual right off the bat.
Sit next to the girl, not across. This allows you to touch her shoulder, etc. more easily while chatting, and it makes the whole thing feel more like a hangout rather than an interview or date.
After 20–30 minutes of chatting, suggest watching a movie at your place. (Pre-plan a low-key movie to watch that doesn't have so much action that it's distracting.)
If you go back to your place, put on a movie quickly, then offer a massage. Sit behind her so your legs are on either side of hers.
After two or three minutes of massage, pull her toward you and say, "Kiss me."
Foreplay.
Sex.
Your date is now excellent.
