What are some good transition steps between hanging out and a kiss?

Well if it's objectively an awkward moment, then obviously don't go in for a kiss. But more often than not that feeling will just be in your head, and she will think that you are quite confident for kissing her out of the blue. In my experience any time is as good as another. To give a recent example of mine: I was sitting right next to this girl in a bar on a couch and conversation was dying out a little. We had been talking for a good two hours and we clicked. So we were both observing the people in the bar for a good minute, when I told her that she'd been quiet all of a sudden and that I wanted to know what she was thinking. She responded that she sometimes enjoys to just observe and get lost in thought. And so I kissed her and sort of cheesily told her that I figured that that might help her 'get lost in thought'.

It's more about how you feel (and ofcourse there has to be somewhat of a connection, though that connection doesn't even have to be all that strong) and then just going for it. Saying 'shhh' or 'wait, I've been wanting to do this for the last 15 minutes' and then kissing her, or not even saying anything at all is good enough.

For every stage of physical escalation you use the traffic light system.

Red (no) - If you go to hug a girl and she pushes you away, or you go for a kiss and she gives you her cheek or asks wtf you're doing.

In this instance stop what you're doing and back off, if its a strong no then you can take a physical step back and apologise and say you're in a good mood or whatever, and then change the subject. Try again later.

Amber (neutral) - If you go to touch a girl and she kind of just lets it happen but doesn't reciprocate or push you away.

This is an indication that she's not ready yet, you don't keep pushing, you back off and try again later. But you don't have to draw attention to it or apologise or anything.

Green (yes) - if you go to touch a girl or kiss or anything and she reciprocates

Very simple, just carry on with the physicality and escalate further while looking out for other signs.

If you get a no/neutral response, you backing off makes her feel safe and reassures her that you're socially aware enough to not violate her boundaries. You can then try again later up to about 4 times I'd say. No matter what you do not let this fuck up your mood, or make you feel like you've been rejected. She's still talking to you, she just doesn't feel comfortable enough to be that physical.

You can power through a lot of bullshit in game, powering through bullshit is a valuable skill, but don't power through the process of physical escalation. No does in fact mean no.

Physical touch my friend.

Youve just spent an evening with her. At this point a kiss shouldnt come completely out of the blue. You should at the bare minimum have been holding her hand (if shes holding yours back, then its a great sign!)

Usually when I meet at the start of the date, I give them a kiss on each cheek (as a polite gentleman) and usually do this with a hand on the elbow. Give here compliment that she looks great especially her outfit (choose your favourite part) then as you release just let your palm brush down her arm and hand

If things are going well later, when walking or talking or drinking - take her hand without breaking eye contact.

Im not advocating forcing yourself upon her. If she seems stand-offish or isnt reacting positively to your evening, then maybe physical contact isn`t a great idea.

In my experience if you are sure it's the right time you can sort of create an opportunity by by just saying "wait". She will pause for a second maybe ask "why/what" and that's your quite moment to move in.
