I'm not defending the guy, but I'll give some insight. Resident doctors work a lot, ~80 hrs a week. Since the guy is internal medicine, he's on call every few days, which means he has to stay the whole night in the hospital without much sleep at all. It's a really shitty lifestyle, and it's really hard to go out when you're a zombie. Add to the fact that you get yelled at all the time by attendings, and you don't have a lot of time to dedicate to the gym, you're self confidence can take a blow.
