Sounds like the parents didn't get adequate info about post surgery care of their infant, which is why the hospital upgraded the pamphlet later. Not saying that makes it all ok, but it's not as bad as your title suggests...

Lack of post-surgery info angers grieving parents. With a baby son dead, they want care after circumcisions clarified.

This was also in 2004, and circumcision is no longer a routine procedure in Canada.
