It's hard to say what's wrong with you based on your post, but here are a few tips:
Talk to everyone. This works to your advantage in a number of ways, but the main benefits are that you create social momentum that favors you meeting new people (it's much easier to roll into a conversation with a new group of people if you're already in one, and you'll likely find new friend groups to hang out with if you keep chatting), you look like a fun, social guy who belongs at the venue, and you have a huge bank of positive interactions to draw from if you get rejected. Think of it like this - if you've had 10 good conversations, and 1 bad one, you'll have an easier time keeping your mood up than someone who's had 1 bad conversation and no good ones, and if you're in a fun, social mood, you'll have a much easier time attracting people to you.
Don't be afraid to switch venues if you feel like the spot you're in is played out. You can generally get a feel for this after a few passes around the location, and a number of factors can contribute to your impression, but generally you'll have a sense of whether the place you're in is conducive to you being in a good mood and having a good time. If it's not, try another spot.
Make yourself comfortable. If you aren't having the best time, stop and check to see how you're feeling in the moment. Are your legs getting tired from standing? Do you not like the angle you're sitting in? Sometimes, the reason your experience is being negatively impacted is because you're doing little things to make your body uncomfortable, and correcting them can bring you back where you need to be.
Play to your strengths. If you're a good dancer, then feel free to hop on the dance floor, but if you're more of a talker, leverage that. Go to areas where you can actually talk and be heard, like by the bar or outside to have a smoke. If it's hard for people to hear you, don't lean in when you talk; instead, retain normal posture and project your voice. If the person can't hear you, then talk normally directly into their ear (and have them do the same if you can't hear them). Do whatever it takes to make your conversational skills work for you.
If you're making genuine progress with a girl you find attractive, don't leave, stick with her and her friends until you're no longer making progress.

How to approach man? Nobody makes eye contact with me as it’s super crowded and I’m not hot enough to stand out. Should I just go up, tap on their shoulder and make my move? Or just speak loudly? I do this thing where I stand in the way or near the bar and when someone walks past me or near me, I try to make a comment and start a conversation. This fails many times but succeeds like a few times. I see girls talking to many guys but when I try the conversations end super fast. I don’t know where I’m failing as I can keep a conversation going well with strangers on dates etc

Few tips:
Approach from the side rather than the front (makes you seem less imposing and more casual)
Make sure you're physically comfortable when you approach
Go places that are easier to have conversations (if near the bar doesn't work for you, try heading outside for a smoke)
Don't just talk to girls, talk to other guys and the staff (this makes it a lot easier to pivot into conversations with girls, makes it a lot more likely you'll meet girls, and also puts you in a better and more talkative mood overall)
When you go out, try to dress at least one level above the other guys (helps you stand out in a positive way)
