If you have been seeing her for a month, you shouldn't be playing UBER-hard-to-get. Sometimes people need to be flexible. If it is a regular occurrence, then it might be time to take action.

On a first date I would definitely tell her you found something else to do after you thought she stood you up and I wouldn't even reschedule unless she made an effort to make it up to me. 15 minutes late isn't a big deal. 1hr30 without even a courtesy call is downright rude and disrespectful. Unless she has a really, really good reason that shows a flaw of character not worth pursuing.

One exception I can think of is if the "date" was some loose "hang-out". Like you said "I'll be at XXX bar at 6 with some friends. It would be cool to see you there". In that case you aren't making plans as much as you are inviting her to something you are already doing. She doesn't think you are waiting for her alone then, you are with your friends or whatever so she is allowed to be flexible. In that case, if I was going to be out until 8:30 I would tell her it is no problem and she can drop by whenever she is free.
