Be decisive, just say "We are doing X, unless you voice an opinion" - that will get the activity moving. But if you don't want to make the decision, make her do it. If she won't, then do something slightly embarrassing until she does. "I want you to decide. And if you don't I'm going to hold in a hug, and softly kiss so you make moaning sounds so everyone passing by can hear. And I'm going do this until you do decide."
Note: I have not done this; this could backfire on you massively.

This happened to me and I just started being more assertive. Instead of saying "where do you want to eat?". Say, "lets go eat at ..." by choosing places that you like. Also, ask her simple questions to figure out her likes and dislikes on random topics like food, sports, hobbies, family, and friends. In time you will learn she likes and dislikes which will make it easy to handle her indecisiveness. With time and getting to know her more it will become easier.
TL:DR; First make the decisions on what you like. get to know her. Then make the decisions that you and her like.

Yeah i've been trying to learn her likes and dislikes. I found out that there's not much that shes really likes or dislikes, and i am the exact same way which makes it more difficult to decide. I'll have to get used to being more assertive though.
