I noticed:

Touched her early on the shoulder, also handshake

That first hug was weak but at least he did it - we're all going to do weak ones as we practice

KILLER eye contact - this was huge. This alone probably made up for the things he was doing wrong.

Clearly showed his intent - also a big one

Forgetting her name isn't a big one, IMO. Often the girl will forget your name as well, and besides it's a pretty natural thing to happen. If the girl is going to be a prickler about it she's probably too uptight anyway.

It was good that he went for the instant date but I think the way he went about it wasn't so good. He was kind of asking/kind of suggesting. I would have probably taken the lead more. Perhaps more experienced sedditors could way in on this.

He knows how to set up a kiss. Got closer to her, slowed his speech, maintain very strong eye contact. I can't tell from the video but I wouldn't be surprised if he was triangle gazing or not (looking from eyes to lips)

One thing I didn't like was how he made fun of short guys - but maybe that's just because I'm short hahaha. Personally I try not to put anyone down to make myself look better even though some more "major leauge" PUA's such as RSDBrad have said they do it from time to time and can yield good results.

Even though he asked "can I give you a kiss" he wasn't really asking, he was already going for it so I don't think that instance counts so much as asking permission. When she rejected the kiss he didn't freak out, he maintained frame really well and just rolled with it. - Harder than it looks.

The "is it too soon to kiss?" I would say is too permission seeking. Don't EVER put liability on the girl. Guys make the moves for a reason. At this point she was so attracted it wasn't going to ruin anything anyway though. Wasn't permission seeking for the make out though, he got the message that he could do it, so good on him. Having the balls to attempt in the first place is a victory
