How do I break the ice?

DaygameCoach has great advice. Talk to everyone. If you're in a service job (a waiter, for instance, I am), talk to your customers regularly. If you go into a store, have a small conversation with the cashier. If you go into McDonald's, talk to your server. You'll slowly start to realise that if you can talk to these people then why can't you talk to a person you like in a bar, a club, even in broad daylight in a park or the supermarket.

The way I approach girls (my friends call me the instigator on nights out because I tend to be the ice-breaker for the group), is I'll walk up, and compliment them first. It needs to be a slightly less obvious compliment, something she was maybe not expecting. For example, if you notice she's got a particular brand of clothing, compliment it. More obvious things will work too, like her eyes, hair colour, but the compliments that she'll notice are the ones she wasn't expecting. Not over the top ('mmm yeah, you're ankle socks are rather fetching'), but show that you NOTICE her.

Then, if she smiles or says thankyou, introduce yourself. TELL HER YOUR NAME FIRST and then ask hers.

Boom. Conversation started.

It won't work every time, of course you'll get rejection, but that's normal. You wouldn't take on every girl that comes onto you, don't expect them to do the same. Also, don't be shy about your appearance. I'm a skinny 6'4" Jack Skellington looking motherfucker, but if you exhale confidence she'll be drawn to it and she'll look past physical appearance (but then, just like guys, there are some incredibly shallow girls out there, don't let it put you off). A massive piece of advice I received is to dress for the body you have, not the body you want. I realised this and promptly went out and bought slim fit clothes, that fitted perfectly. No designer stuff, no fancy stuff, just clothes that FIT me.

Besides, what's the worst that can happen? She'll say no? Big woop, right?

Get it out of your head that it is creepy to approach or even that it is random. Its creepier to be that guy who really wants to approach but doesnt. Just approach. And talk to everyone...in your classes, the halls, campus, bookstore, cafeteria, etc. Be that social guy and build social proof. You can also join campus clubs. Maybe you need to stop drinking so you can get used to approaching sober. Stop telling your self you cant do things or that you are too shy or that it is creepy...those are self limiting beliefs.
