That album was mainly about battling his addiction to heroin.

Hence all the self loathing imagery.

Hurt is all about the numbing effects of heroin and how it takes away your ability to feel any sort of emotional pleasure

Submition statement:

There are allot of similarities of Reznor's journey in this album to those of a red pill man.

God - society/power
Piggy - oneitis/others who have control over you
Ruiner - the destructive tendencies of man (self-destructive), most often caused by those who are slaves to their disires/feelings

He begins his journey as a standard blue pill man, unable to love or be loved without hurting or being hurt. He becomes fed up with that and becomes detached in order to escape the pain. He escapes detachment only to become abusive towards the external world witch has hurt him so. When that doesn't work he starts to realize that he himself is the source of his pain.

This is where the journey diverges from the per pill man. In the end where he doesn't commit suicide (metaphorical one) to be free. This conclusion is supported by the final lyrics:

"i wear my crown of shit// on my liar's chair// full of broken thoughts// i cannot repair// beneath the stain of time// the feeling disappears// you are someone else// i am still right here"//

"if i could start again// a million miles away// i would keep myself// i would find a way"

He has not conquered/transformed himself (committed suicide (the boy must die so the man can live) ) and is unable to witch is the source of his pain and his chains. His cowardice has doomed him to a life of suffering.

tl;dr: A man must conquer society to become powerful, but he must also conquer himself in order to become free.
