The following should be instated, it would be fair to both men and women:

Required documentation on intent to be a parent. Before a child is born, it should be documented that a couple has mutually agreed to become parents. In the case a sperm donor is used, a certificate and dna sample from the spermbank/clinic could be used as replacement.

Mandatory paternity tests after birth. This is less than $100 in a lab now. To see that everything is that it should. Aside from emotional turmoil prevention it is also good for the child, in case it ever gets in need of organ or blood donation to know that "Dad" is also the father.

If it is revealed anything is wrong with the above points and deviates from how things were supposed to be?

The kid is forcibly adopted away just after birth. There are so many prospective good adoptive parents that this should not pose a problem.
