First, prostitutes likely vary greatly by location. Often they probably are safer than just picking up random girls in bars.

However, there aren't really any guarantees. Condoms aren't 100% effective but in general they do protect against a lot of diseases. That does not include orally-transmitted HSV as others have pointed out. It also doesn't include HPV which men generally are not vaccinated against.

You mention miramistin. I couldn't find a lot of solid info on that online and as far as I can tell it isn't FDA-approved. That is a bit of a red flag as it probably hasn't gone through rigorous clinical trials. It might very well be safe, and it might help, but I wouldn't count on it as some kind of get-out-of-jail-free card.

If you must have sex then you're going about it in the right way. However, I personally don't consider the risks worth the benefits, even if those risks can be minimized.

Brothels aren't bad here in Australia but they can get expensive. Basically you pay for your time (half hour, full hour or more), choose your girl and off to your room. From there you talk to the chick and work out anything extra. Anal, oral, whatever kink you have.

The girls are all tested regularly for diseases, cause that's the law, obviously. But there's still a chance, as always with any girl nowadays.

Once you use one you should be fine, just keep your wits about you. If you don't feel comfortable about a girl, go with what you think what's best for you in the situation. You're overthinking the experience I believe.
