In direct answer to your question, you need to visit this site. This is the official court repository for official court forms. Scroll down to "family law." Now, none of us know what's been done in your case. We don't know what's been filed, what the status is, what the "contested" issues are.

If nothing's been done, you are going to need at least some of those forms, guaranteed. But that's not all. Family law court is a civil court. So all of the regular civil forms and motions and practices apply too. This includes things like discovery, if you have a discovery dispute. Though the civil law family forms will get you a good bit of the way. Or, maybe all of this is done. Maybe discovery is done, some of these forms have been filed, etc. But we don't know. Only you know.

What you linked, the Northwest Justice Project, is reasonably good with their guides -- but they are basic. Good, but basic. Be sure to visit their trial page. Their information is correct. But if you are facing a lawyer on the other side, they are going to have experience and case law knowledge. You'll need to be very well prepared to counteract that.
