In the latter they are truly happy and feel wanted, but it is a momentary happiness with an equal and opposite dose of guilt and self hatred coming later.
This is why you'll get the strange comments in a relationship like "all you care about is sex" and "why don't we ever do anything romantic as a couple", it's actually projection of her inner feelings of guilt, and her hamster spinning up wondering what you've done for her in return for that guilt lately.
