Greater Male Obesity Risk
In 2016, The Global BMI Mortality Collaboration30175-1) released a meta-analysis of 239 prospective studies across four continents examining the relationship between body mass index (BMI) and all-cause mortality.
The primary analysis focused on never-smokers without prior disease who survived at least five years. Both overweight (BMI 25–30) and obese (BMI 30–35) individuals were associated with increased all-cause mortality, and this relationship was steep across all global regions, except South Asia, where fewer deaths were recorded.
Here were the sex based findings:
- At equivalent BMI levels, men exhibited greater insulin resistance, higher ectopic fat accumulation, and a higher prevalence of type 2 diabetes compared to women.
- Notably, a given increase in BMI was associated with a far greater absolute increase in mortality among men
- The absolute excess death rates associated with grade 1 obesity (BMI 30-35) was about 3 times greater in men than in women!

Visceral Fat differences between sexes
Other factors, such as visceral fat and fat distribution, were not included in the Global BMI Mortality Collaboration analysis, these are not accurately measured by BMI alone.
An influential review by Després (2012) described how visceral adiposity, the fat surrounding internal organs, is metabolically more harmful than subcutaneous fat. It is strongly linked to atherogenic metabolic abnormalities, insulin resistance, dyslipidemia, and cardiovascular disease.
This harmful pattern is more common in men due to their tendency to accumulate more visceral fat.
A meta-analysis on children by Ortega-Avila et. al (2025) found that sex differences in visceral adipose tissue (VAT) accumulation become more pronounced during puberty, suggesting that biological and hormonal factors drive these differences from a young age.

Sex differences in obesity risk
A study by Kim et. al (2025) confirmed that men are more susceptible to visceral fat accumulation, which further increases the risk of cardiovascular disease (CVD), type 2 diabetes, and other obesity-related complications.
The study also identified distinct gut microbiome profiles between obese men and women. It also found distinct gut microbiome profiles between obese men and women.
Men were found to be more susceptible to earlier onset of CVD and diabetes, and obese men face more severe metabolic complications due to differences in fat distribution and hormonal profiles.
The authors emphasized the importance of understanding sex-based differences in obesity, particularly in guiding prevention and treatment strategies.
Sex Differences in Cardiovascular Risk
The Framingham Risk Score (FRS) is a validated tool used to estimate an individual’s 10-year risk of developing cardiovascular disease (CVD) based on factors such as age, sex, blood pressure, cholesterol, smoking status, and diabetes. Studies consistently show that men have higher FRS scores than women, even when controlling for all traditional risk factors.
It is done based on the decades long Framingham Heart Study.
This indicates that male sex itself confers additional susceptibility to cardiovascular disease. For eg., at comparable levels of blood pressure, cholesterol, and BMI, men tend to reach higher risk thresholds earlier in life, reflecting both biological differences in fat distribution and hormonal profiles as well as possibly greater cumulative exposure to cardiovascular risk factors.
Just look at the tables, they are literally separated by the sexes first and foremost!

As you can see even for equivalent men have a 3-10 times greater risk of heart disease!
In the context of obesity, this means that men not only accumulate more visceral fat, which drives metabolic complications, but they are also at biologically higher cardiovascular risk, compounding the overall health burden that is associated with excess weight.
Conclusion
Obesity and cardiovascular diseases hit meny completely different as well as are more harmful. It is a completely different killer and more awareness must be raised about this.
Exercising and staying lean is much more important for men than women.
