I'm going to assume you include cosmetic surgeries as well. There are a few cosmetic changes I'd like done but it wouldn't make much of a difference other than to sate myself.

I wouldn't have surgery to appear masculine. I would have surgery to look better in my eyes.

Contrary to popular belief in here, yes, plastic surgery can definitely make one's face appear more masculine if done correctly. Example of jaw augmentation.

The fact is, all you people suggesting "lift weights and get a new haircut" don't realize that your facial appearance is extremely important, and your facial structure can only be changed via plastic surgery.

I'm curious as to what you would fix? Add something to your chin or cheekbones? Get muscle implants (lol), or something like "Anchor Arms" from Spongebob?

I think reconstructive surgery should be reserved for broken noses, cleft palates etc.

I have an aquiline (or "Roman") nose. For the longest time I wanted to get it fixed. Now, I've begun to grow into it, and it's actually quite masculine-looking. Even if it's different, I don't care enough about my nose to fix it. I think that fact is infinitely more attractive than a little button nose would be.

If you're worried about jawline, cheek bones etc, lift, eat protein and drink a lot of milk, for starters. Maybe check your testosterone levels? Women have a wider range of what's considered physically attractive.

Perhaps with advances in epigenetics, we'll be able to chance more about our appearances. Work on confidence first though--or preferably, work on both.
