Hormonal decline, in incredibly rudimentary terms - the chemicals inside of a person that make it easier for them to stay fit/in shape/attractive hit a drop-off point (even accounting for genetics and good habits) at different ages and rates for the sexes. For most men it can be a slow ebb or even plateau with hard work. For many women, the drop-off is steeper.

So why don't women engage in hormone replacement therapy? Dudes have TRT, so my half-baked understanding of endocrinology suggests that ERT would help 30-somethings push the wall back a bit further.

I'm not sure why they don't, actually. Maybe there are unintended consequences that science hasn't yet figured out how to negate. Maybe it feels like "cheating" (I am just purely speculating, not saying that's how women think). Maybe many women are happy to age out of attractiveness for a variety of reasons - here was a thread linked to a few days ago that covered just that.

Without passing judgement, note that many of the responses reflected less of a concern about maintaining attractiveness after having found stable provider/mate (this is also true for some American middle-aged men as women)
