Honestly I'm not seeing a huge problem. This happened before you guys were exclusive. She did say that white lie but it was probably to spare your feelings. It's up to you to decide if that was a deal breaker or not. If so then leave, if not then don't mention it.
Definately do not say you went through her phone. You would be viewed as insecure and needy and will look bad on you

What are your plans and hopes for this? Straight up. No judgement if you love this chick, it's just important for you to be honest with where your at, knowing will help you be more prepared for what you have to do. (And that is all up to you. You can stay and not mention it, or you can start a fight, or you can straight up leave and look for other chicks)
She basically cheated on you and didn't tell you. It's up to you what boundaries you set for your mono-LTR (if that's what this is).
You know about it and you aren't punishing her for it. I wouldn't be incredibly shocked if it happens again or happened more than once.
Not explicitly trying to sew fear into your perspective, just telling you what I think.
If I'm being harsh here I'd say confront her, tell her you know, tell her she's hurt you and it's not okay and that you need some time apart and just walk out for a few weeks. Go hit up some other girls, see if that feels cool. Come back and pretend nothing happened if you want to but don't expect her to magically change forever. If she did it once she'll probably do it again. Or conversely, do something fucked up to her and break up with her if you feel like being a dick.
