Approach anxiety is an irrational fear that has to be overcome. The fear stems from a fear of rejection and fear of bruising your ego. The only way to get over this is to dive in face-first and take your lumps like a man. Every approach is a victory, because it means you faced down your fear and powered through it. Eventually, you'll see that there's nothing to fear. Being rejected doesn't physically hurt, and the women who reject you aren't shaming you with a bullhorn while the entire room points and laughs at you.

As this fear subsides, you'll relax more and you'll have more success, because women respond better to the relaxed guy than the stutterer that's white-knuckling his beer and is afraid to make eye contact. Confidence comes with practice. You'll get better as you discover what icebreakers work and you refine your game. Remember: guys who do well with women get shot down too - the difference is that they know it's part of the game and they just roll with it and move on to the next opportunity.

If you don't approach, you'll never get what you want. If you don't get what you want, you'll never be happy. If you're never happy, then what's the point?

Let's start with this. Your title. Not the slightest idea? You can't seem to fathom any possibility in lack of confidence? Don't like your physique? Too short? Not muscular enough? Voice? Perhaps something less personal and more material - Don't like your car? Ride a bike maybe... What is it? Beige Phillip Rule #5.9 Whatever is broke - fix it. You can't finish a race if you don't even know where the starting line is.

I have no courage or confidence

As as cliche as it sounds, attitude really is everything. I went through your post history and see that you focus a lot on the negative sides of things. Picture your mind as an open field for harvesting your subconscious is the farmer. If you keep giving your farmer negative seeds to plant, what the fuck is going to happen? You're not going to have crops you want sunshine.

If you had a broken car for 6 months and didn't try to fix it, you can't complain that it won't get you to work.

If this is true, examine them. What's their routine like.. How's their language? Take a real good look at them and their life. Ask yourself why are they confident and what qualities do they process that fuels their confidence.
