The black pill is getting out of the cave and staring at the sun(truth) untill you burn out your retnas and become blind.

The black pill is basically nihilism/pessimism/utter loss of hope/etc. I've also heard it used to describe a particularly devastating redpill. It can be applied to any context, just as people have used the term redpill to describe realizations aside from male sexual strategy. So it should come as no surprise that incredibly ugly incels would say they're black pilled, but I don't think the term is limited to that group.

I think it has less to do with the content of the pill and more about how the individual processes it.
