Yes, greet her with a hug and make sure to wear nice cologne.
At some point during the high point of the date where you are standing next to her and you guys are laughing or you find her cute or attractive give kiss her on cheek. If she looks towards you after the kiss she wants to kiss you. Always make physical contact during the date like touching her arm while explaining something or explaining something. Break the barrier, be confident and take initiative. And if she says no to anything be cool and don't let it phase you too much
Hold hands only if she offers. It's more of a couple thing to do.

Dating is all too contextual and nuanced and you have given too little information to go on. Are you guys Mormon or other conservative fundamentalists? Are you secular living in a big city? Where you going?

Some general ideas - -Have a plan. And a backup plan. She should be included in decisions but not have to make a decision. -keep it lite and fun and inexpensive. Coffee or bottle of wine in the park with a loaf of bread beats a steak house -bring a mindset that you are there to see if you feel attraction to her, not the other way around. Ask her questions, listen to her, don’t be concerned if it doesn’t work out. This is an experiment, not an interview where you need the job cuz your broke. Believe you have other options if she doesn’t make you want I see her more. -if you do get the feelz and she doesn’t, it’s no big deal. Dating is a numbers game and someone else will catch your eye soon. -have a few interesting tidbits about yourself to share.

Always go for a hug at the beginning of any date. Act as if she's a good friend of yours and it won't seem awkward if that's what you're worried about.
I find that to be a bit lame. Ideally, you should be kissing her on the lips at some point during the date tbh. Just find a moment where you are relatively secluded and looking at each other in the eyes intently and then go for it. Usually it takes at least 30 minutes to get comfortable with each other before an opportunity like that will arise.
Holding hands is also a bit lame, but in some contexts it might work. I would only hold hands with her after you have already kissed her though. But yea, might be a bit too much. Save it for the 3rd date tbh.

The answer to your questions is the same. Human beings are very receptive to touch. Honestly whether you get a second date or not is very dependent on how you touch her. You should generally try to escalate from least intimate to more intimate touching over the course of the date. High 5s, hugs, touching her arm, brushing against her hand, leading/guiding her, offering your arm for her to hold are friendly touches. Putting your arm over her or around her, knee touching, her legs over yours, are mid level touches. Touching her face, handholding, kissing and having her body against yours are intimate level touches.

Escalate but be sure to gauge if she gets tense or looks uncomfortable and back off if it happens. Try again a little later.

Make no mistake, touching is, imho, the most important part of dating and why so many dudes get friendzoned.

As for going for the kiss, if you follow the natural progression I've laid above, you'll always lead to a perfect kissing moment. When you get to it, you will know. Make sure you don't hesitate.
