This happened literally 30 minutes ago. First time I try to game I fuck up by not introducing myself properly.

So today I was late to my college physics class, and I took the only empty seat, which was next to a cute girl. I started small talk with her, discussing about the quiz questions, talking how shitty was the recent exam, and how much I suck at physics. She seemed chill with that and she also started talking about the same stuff.

Towards the end of the lecture I go "I'm sorry, I didn't introduce myself. Hi, how you doing?" Her (giving a small giggle): "oh hey, my names abcxyz."

I ask her if she's "in a sorority, cause I swear that I've seen you around somewhere."

Her: "yeah I'm in xyz sorority"

So I tell her about how one of her sisters was in my class the previous semester, blah blah, and tell her that I'm also in the ABC fraternity.

The lecture then ends, I told her that it was nice meeting her and I hope seeing her in the next lecture, and then scurry out of the class cause I had a class right after (and I was getting anxious too, fuck the shy me).

I just realized that I didn't even tell her my name and just hurried out of class. WHAT THE FUCK IS WRONG WITH YOU, 1NSANE95?

In my defense, I'm a shy dude and have low self esteem because of my body weight.

Tl;Dr: spoke to a girl, fucked up by not telling her my name.

How do I not fuck up the next time and just talk straight without getting flustered?
