Talk to everyone in the gym. There will be certain people who go at the same time as you more than likely, ask them for a spot and befriend them. If you are the guy that just looks like you talk to everyone then it'll be easier for you to talk to the girls who go there at the same time as you as well without giving off a creepy vibe. From my experience I have fucked girls who go to the gym with me but I never really hit on them there. I would end up seeing them out somewhere else or on our walk in/out of the gym. Maybe you catch them outside of the gym and say hello or something. But if you get a harsh rejection then it's going to not be fun going back to the gym the next couple of days. Just my two cents on the topic, I'm sure other have different opinions.

I met my BF at the gym. Go for it. As a female, its great when a guy is interested. if you are smart and in good shape....you sound like a winner!

Yeah, I have this issue myself. There's a girl at my gym who goes literally every day and I'm almost always there when she is there. She sometimes makes incidental eye contact, and has even sat on a bench next to me, but have never talked. On the one hand it's good that I'm focusing on the workout, but I do feel that maybe I should just ask her "Hey, I've seen you here all the time. You look like you're training for a competition."

So I go to a large chain gym in a big city. The ratio of guys to girls is probably 3:1. Naturally, girls are on their guard and many have headphones in etc to ward off the orbiters.
So, realize that they are signalling their level of openness. If they're there to work out and get out, leave them alone.
Specifically, you don't have to be afraid of them. You can make eye contact; you don't have to avoid it. You can smile; nothing requires you to be a storm cloud.
And overall, you have the wrong idea about "seduction." It's just socializing. Meaning that you are using the same basic skill set whether you're talking to an old lady, a guy, some girl you're not attracted to, or that "hot girl."

But a serious question: if a girl is making frequent eye contact, is it okay to approach? Or is too taboo to make a move?
There's no rule for this. You've also said nothing about whether she has been smiling or not. You've said nothing about whether she had headphones in or not.
If she wasn't smiling or had headphones in, then maybe she wasn't looking to talk.

I have always steered away from the gym pick up game since it's such a hyper masculine environment where girls aren't really comfortable, but maybe you all have different experiences?
Here's what I'd recommend.
Stop thinking about this in terms of just getting this girl or that girl. It's SOCIALIZING. So with that in mind......
Talk to the staff at the gym. If you aren't on first name basis with them already, you're kind of fucking yourself. (The last time I hooked up with a girl from the gym, she joined a conversation I had started with the gym staff.) Ask them how their day is going, as you're swiping in or whatever. Get to know them a little. Even if you aren't interested in them, it leads to other things. Social networking.
Interact with others at the gym. Make eye contact. Smile. You don't need to be pushing conversation. Some people are there to get a workout finished, as are you. But if someone doesn't have headphones in, you can drop a throwaway comment about something that happens to be on your mind.

What do you recommend for throwaway comments? I feel like clothing isn't a good choice since....well....everybody here wears yoga pants so

You'll notice that I said, "on your mind."
The best approach is not to adopt some suggested throwaways, but rather to observe and make comments if something stands out.
Or, "Hi."
Point is, the comment will get a response to her. Which you pay attention to. That response will tell you how receptive she is to talking. If she wants to talk, "Hi," is sufficient.
