A tip I would suggest is not be as bland for your first statement. I avoid automatic responses I.e. How are you doing? What do you like to do for fun? People get asked those questions everyday and they are boring.
My suggestion is to approach her and find something unique to say. If all else fails look at the environment. If it's in a gym ask her how she got muscles big and of she can give you any pointers. I always start with a joke. If she is listening to music listen to see if you can tell what song it is and scream "Oh my god that's my song! Is that Wrecking Ball? Can we share?"
It really depends on the situation. Girls at the gym get creeped out easily so try to be funny first and a bit less direct.
