You get into friend zone in 2 cases:

She's not attracted to you and either wants to let you down easily, or genuinely wants you as friend.
She's attracted to you but you become a fixture in her life before getting together. Since you gave yourself up so easily, she doesn't want to risk losing you by going sexual. For us sex is what matters the most. Women want to create an emotional connection and they want someone they can rely on. If you become her rock without making sure that you're going sexual, she's gonna friend zone you.

K so how do you become a fixture in her life? By spending time with her regularly (a couple of times a week), constantly texting her, constantly helping her, getting to know her very well, essentially acting like a boyfriend before being one. If you see this girl like once a week, chit-chat a little, hardly ever text and most important of all - you flirt, then you should be safe. You're in a nothing-zone of you want. You haven't become a fixture in her life so she hasn't come to the point where she needs to decide whether she sees you as a friend, boyfriend or lover.

So how do you fix this? If it's the first one, you can't really, unless you back away and she really, really wants you as a friend. I've seen cases in which a girl who wasn't really attracted at first went mad when the guy backed-off and eventually accepted to go sexual with him. But those are so rare.

If it's the second one, well yet again, it's unlikely that it is friend-zone, but if it is, back away a little, let her wonder, when you see her make it sexual, ask her out and you should be fine. So you have to determine whether she likes you or not. If she flirts back, that's usually a good sign.
