It depends, though legally it's up to the mother. The child usually takes the last name of the father listed on the birth documentation, but you can't force this, even if the child is yours and you acknowledge that the child is yours on the birth form.

The Supreme Court ruled that illegitimate children’s use of the father’s name is discretionary. Even if the biological father acknowledges his illegitimate children, he cannot force them to use his surname.

You're the man. You tell your Gf that "My children will have my last name". That's all there is to it.

You can't hold her to it, but she can't hold you into raising children who don't have your name.
