PSA isn't a good screening test for prostate cancer. Most guys with raised PSA don't have clinically relevant prostate cancer. Not all guys with clinically relevant prostate cancer have a raised PSA. Testing on a population basis doesn't lead to reduced prostate cancer mortality. Until we get a better screening test there's still more risk of harm in asymptomatic testing than there is of saving lives.

I would absolutely advocate getting PSA and clinical examination if you start getting any prostate symptoms like difficulty passing urine, reduced flow, getting up overnight, or having to rush to the toilet to pass urine suddenly.

Also get any problems like unintended weight loss, unexplained back pain or blood in your urine or semen investigated urgently.

MRI is more sensitive and more specific than either PSA or prostate biopsy for detecting cancer, though that doesn't seem to have led to much advocacy for MRI screening yet.

But there is no other screening test and screening tests are essential. I am a retired professional prostate pathologist. It isn't rare, prostate cancer is extremely common especially in older men. 13% of men will get prostate cancer at some point in their lives.

Only 15% of prostate cancers can be watched. 85% require aggressive therapy.

My brother in law has a small cancer which is grade III/III so he can be watched. My cousin has a IV/IV which is clinically palpable. But the PSA picked it up. He required radical surgery but still has a 30% chance of dying of his disease.

I really want to see a reduction in the death rate from prostate cancer in my lifetime.
