When you first see her, say "Hey, long time no see!" and go for a hug (or side-hug if she looks uncomfortable, or even a handshake) just to break the awkwardness.

Expect her to look even worse in person, it's typical.

You don't want to feel like you have to entertain her. For her it's a feeling out kind of deal, it is for you as well! Just do your thing and talk what you feel like talking about. This post came across as needy to me.

Don't focus on entertaining her, focus on having fun yourself but being interested in her. So ask her lots of questions, laugh at funny things but be natural. It'll probably be a bit awkward at the start but that's the nature of the beast.
