Is the movie date to a movie theatre or at home? If it's at home just have her lay on your lap. If you're in a movie theatre, there's nothing wrong with the cliche "grabbing her hand in the middle of the movie."

Get yourself something like these:

Gives you a good inch or more but they don't look at all like you're wearing shoes meant for that purpose! (I have the black ones (I scuffed them up a bit and they look really cool) and I'm 6'4").

You are also fortunate that shoes with thick soles are in fashion at the moment (google the shows from london fashion week...) Or go on GQ etc etc so you could go for something even more bold.

All you can do is that and then own it. It's not a big deal seriously don't worry about it.
