Hey mate - I think the important thing to do is stop treating this like a date - You're putting a very large amount of pressure on youself.

At this point, the best way to look at this, is catching up with a friend. You don't have to kiss her, you don' thave to hold her hand, you just have to hang out together alone, with the knowledge that you're both into each other. YES eventually you'll need to start pushing things and flirting more, but to start, just take the 'date' pressure off - This doesn't need to be anything more than a conversation amongst two people. Throw in a compliment so she knows you're still into her and leave it at that. Overcomplication at times like this is a killer.

From what i read I think you might be worried about having an awkward date and having those weird awkward pauses which kills the vibe.

Hopefully this might ease your nervousness a little.

She said yes and is obviously interested, she is going to be nervous too (every human is nervous prior to a first date and every human forgets that their date is just as nervous as them...REMEMBER THAT)

Ok so now onto conversation. You will be awkward if you guys are talking about stuff that you are not interested in. So make sure the convo is about something you like. I guarantee you will loosen up and you will have stuff to say about something you are interested in.

I was on a date once with a girl and it was awkward for a little bit until i found out she loves scuba diving and i LOVE scuba diving. So now you each have something you both mutually like and you can bounce off of each other. Also keep good eye contact (not creepy but make sure you look her in the eyes a good majority of the time) Good eye contact shows that you are interested in what she has to say and you want to know more. Plus if you have pretty eyes she will probably melt.
