How do I tell a girl she’s annoyed me

I guess I would say something like "Hey, I like hanging out with you, but you're making it difficult to make plans. If you want to see each other again let me know, if not that's fine too." and then don't think about it anymore.

People make time for what they care about, so maybe she's just not that into you. It sucks, flakiness is very annoying.
