My observation, the most common hesitation points I have found are the move to go over/under the bra and start touching bare breast and touching the P, first over and then under the panties.

One tactic to overcome this is to kiss that spot at the hollow of the neck that usually results in that little gasp/sigh and shudder just before escalating to the areas of interest.
