I would say absolutely do not ask her. That would be a major mood killer for most, and I don't think it holds any water anyways. She's free to change her mind at any time, even in the middle of sex, so I highly doubt any judge would even consider that anyways.

I understand your fear of having faced a false accusation, but sometimes you just have to put yourself out there. Most women aren't looking for victims to falsely accuse of rape, and even if she did it, if you don't actually rape her then there is a good chance you will be protected.

EDIT: Relationships, and even hook ups, are really built on trust. If you ask for a consent form it shows you don't trust her, and it also shows you are insecure, which women generally hate. If you really think that she is planning on falsely accusing her then don't have sex with her, but if you trust her, then you just need to take the risk and put yourself out there IMO. After all, she's taking risks, too.

There's no point. Any form you draft up would be thrown out in court and I doubt a lawyer will tell you any differently.
